Our Wellbeing Support service is designed to help people remain independent, healthy, and connected within their local community. We recognise that everyone’s situation is unique, so we take the time to get to know each person — focusing not only on the challenges they may face, but also on what matters most to them and the strengths, skills, and resources they already have.
We provide support in two key ways.
Firstly, we carry out strength-based, needs-led assessments, either in a person’s home or within a community setting. These assessments are more in-depth than our Information, Advice and Guidance (IAG) service. While we may explore similar topics, Wellbeing Support is far more personalised and person-centred. We will arrange a dedicated appointment and take the time to talk things through in detail, ensuring individuals feel heard, understood, and supported.
Our assessments look beyond immediate concerns to build a full picture of a person’s life, priorities, and circumstances.

General information, advice and practical support
This holistic approach helps us understand what support will make the greatest difference to each person’s wellbeing.
Secondly, we offer ongoing, person-centred support, working closely with individuals and the people who are important to them including family members, friends, carers and other professionals. By putting the individual at the heart of every decision, we work together to co-produce support plans that reflect their wishes, goals and personal circumstances.
Our aim is to empower people to make informed choices, build confidence, and feel supported to live the life that matters to them.
